**WAIVER OF LIABILITY AND ASSUMPTION OF RISK

In consideration of being permitted to participate in Brazilian Jiu-Jitsu classes and related activities at Mata Leao BJJ Halifax, I the undersigned participant, acknowledge and agree to the following

1. **Acknowledgment of Risk**: I understand that participation in Brazilian Jiu-Jitsu and its associated activities involves inherent risks, including but not limited to serious bodily injury, permanent disability, paralysis, and death. These risks may arise from my own actions or inactions, those of others, or the condition of the facilities and equipment

2. **Assumption of Responsibility**: I voluntarily assume all risks associated with my participation in these activities, whether known or unknown, and I accept full responsibility for any injuries, damages, or losses that may occur as a result of my participation

3. **Release of Liability**: I hereby release, waive, discharge, and covenant not to sue Mata Leao BJJ Halifax, its instructors, staff, and any other affiliated individuals or entities from any and all claims, demands, actions, or causes of action that may arise from my participation in the activities, including but not limited to claims for negligence, wrongful death, or any other claim

4. **Indemnification**: I agree to indemnify and hold harmless Mata Leao BJJ Halifax, its instructors, and staff from any loss, liability, damage, or costs that may incur as a result of my participation in these activities

5. **Medical Consideration**: I affirm that I am in good health and do not have any medical conditions that would prevent my safe participation in Brazilian Jiu-Jitsu

6. **Governing Law**: This waiver shall be governed by the laws of United Kingdom, and any disputes arising hereunder shall be resolved in the courts of United Kingdom

By signing this waiver, I acknowledge that I have read and fully understand the terms of this agreement. I am aware that by signing this waiver, I am giving up substantial rights, including the right to sue. I am signing this waiver freely and voluntarily
